What Is Virtual IOP?
Virtual IOP is a type of intensive outpatient program that is conducted via a secure online videoconferencing platform. This type of treatment is also sometimes referred to as online care, remote treatment, telepsychiatry, or telehealth.
Before we answer the question, is virtual IOP effective, it can be helpful to quickly review a few basics about virtual treatment in general.
History
Using technology to connect patients with mental health treatment is not a new concept. As reported by the American Psychiatric Association (APA), a treatment center in Nebraska first explored the option in the late 1950s.
Virtual treatment expanded through the years as the technology improved, but the approach received perhaps its biggest boost in the early 2020s when the COVID-19 pandemic temporarily limited in-person gatherings in many circumstances.
Though pandemic-related restrictions have long since been repealed, virtual IOPs and other forms of telehealth have remained as viable and effective options for many patients.
Disorders Treated During Virtual IOP
Any disorder that can be treated via psychotherapy (talk therapy) may also be addressed in a virtual IOP, but the scope of this type of treatment can significantly vary from one center to the next.
Disorders that are frequently treated in at the virtual IOP level include:
- Substance use disorders (addictions)
- Anxiety disorders
- Depressive disorders
- PTSD and other trauma-related conditions
- Dual diagnosis (co-occurring addiction and mental illness)
Benefits of Virtual IOP
The APA page that we linked to earlier states that virtual treatment is equal to in-person care in several areas, including diagnostic accuracy, treatment effectiveness, and patient satisfaction. The page also notes that the virtual option can save time, money and other resources.
Other potential benefits of virtual IOP include:
- The online option offers greater access to care for patients who lack transportation or have mobility concerns.
- Elimination of travel to and from the treatment center makes it more convenient for patients.
- Patients may feel more comfortable attending sessions from the safety and comfort of their own homes.
What Happens in Virtual IOP?
As the name implies, virtual IOPs are designed to provide the same types of services that patients can receive through in-person intensive outpatient programs.
Typical features of an IOP include:
- Treatment is in session for two or three hours per day.
- Patients attend treatment two to five days per week.
- Group therapy is the main method of treatment.
- Other therapies and services also offered.
Services that are provided in virtual IOPs include:
- Group therapy: During virtual group sessions, you and a small number of other participants can gain valuable information about the challenges you’ve been facing, practice healthy communication and problem solving skills, express your insights and learn from the experiences of others, and discover the power of shared support.
- Cognitive behavioral therapy (CBT): CBT is a short-term, goal-oriented form of therapy. It is based on the view that psychological distress results in part from unhealthy beliefs, automatic negative reactions, and maladaptive behavior patterns. CBT sessions help you to identify these concerns in your own life, then replace them with healthier and more productive ways of thinking and acting.
- Dialectical behavior therapy (DBT): DBT is a structured, evidence-based approach that focuses on helping people improve their skills in the areas of mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness. DBT sessions can help you do a better job of handling difficult situations, resolving conflicts, and accepting your emotions without self-criticism or reflexive reactions.
- Trauma therapy: Untreated trauma is unfortunately common among people whose lives have been disrupted by mental health conditions and substance use disorders. Trauma therapy sessions are safe and highly supportive environments where you can process difficult moments from your past, address how they have influenced your life, and eliminate their ability to cause continued distress.
Is Virtual IOP Effective?
Is virtual IOP effective? The results of two recently released studies indicate that it can be.
Virtual IOP for Addiction
In December 2024, the journal Healthcare (Basel) published a study on the effectiveness of virtual IOPs for people with substance use disorders. This research effort involved more than 2,000 patients who had completed a virtual IOP at a treatment center in the San Francisco area between 2021-2023.
The study’s findings included:
- Nearly 80% of participants completed at least 30 days of care and maintained at least 30 days of consecutive abstinence during treatment.
- Virtual IOP participants engaged in effective treatment practices across individual therapy, group therapy, and asynchronous phase work.
- Those who finished the IOP completed over 70% of the asynchronous assignments
- Participant engagement in the virtual IOP exceeded engagement estimates in conventional in-person IOP care (which averages about 50%–68% at 30 days).
- Nearly 45% of virtual IOP participants demonstrated a successful response to care and no longer required IOP treatment.
The team that conducted the study concluded that their results “support the feasibility and effectiveness of delivering a telehealth IOP for [addictions].”
Virtual IOP for Mental Health
A May 2025 study in the Journal of Medical Internet Research found that a virtual IOP can be beneficial to individuals who had been struggling with anorexia, bulimia, and other eating disorders.
The study, which involved 270 patients, also demonstrated that the gains made by virtual IOP participants weren’t limited to disordered eating.
Conclusions from this study included:
- Virtual IOP patients had similar outcomes when compared to those who took part in an in-person IOP.
- Virtual IOP patients showed greater improvements in depression and suicidal ideation than did their counterparts who attended in-person IOP sessions.
- Virtual IOP patients had a higher average age and greater greater racial and ethnic diversity than did those who received in-person care.
The final bullet point is significant because of concerns that the technological requirements of virtual IOP might serve as an obstacle for older patients as well as members of various minority groups. The May 2025 study seems to dispel those concerns, though the authors noted that more widespread research is needed in this area.
